What is coronary heart disease (CHD) and how does it develop?

In coronary heart disease, the heart is no longer supplied with blood properly. At least one of the three coronary arteries that supply the heart muscle is narrowed or completely blocked. As a result, the blood can no longer flow freely through it. Coronary heart disease is usually caused by arteriosclerosis (hardening of the arteries). Elevated blood lipids (cholesterol), smoking, high blood pressure, diabetes, obesity and other factors increase the risk of the disease.

How does coronary heart disease manifest itself and how is it diagnosed?

Typical symptoms of coronary heart disease are a feeling of tightness in the chest under stress (angina pectoris), occasionally with radiating pain in the arms, neck, back or upper abdomen. These symptoms subside within minutes of stopping the exercise. If an acute vascular occlusion occurs, the symptoms persist (myocardial infarction). However, coronary heart disease can also occur without symptoms ("silent ischemia").

If coronary heart disease is suspected, evidence of a circulatory disorder can be provided in a stress test (ergometry, stress echocardiography, scintigraphy or MRI). The diagnosis of coronary heart disease is then made using imaging procedures such as computer tomography and coronary angiography.

How is coronary heart disease treated?

The treatment focuses primarily on the risk factors. As a patient, you will learn how you can prevent your disease from getting worse. We treat some risk factors, such as high cholesterol, high blood pressure and diabetes, with medication.

Sometimes interventions are also necessary. These include balloon dilatation and the insertion of stents into the narrowed coronary vessel or a bypass operation. This involves bridging narrowed areas with a so-called bypass. Both methods restore blood flow to the heart muscle.

In addition to treating the symptoms, correcting a circulatory disorder of the heart also has a prognostic significance (prevention of heart attack, cardiac insufficiency and premature death).

Heart attack

A heart attack is caused by an acute blockage of a coronary vessel. A rapid reopening of the vessel by catheter intervention (balloon dilatation and stent implantation) must be attempted in order to minimize the extent and complications of the infarction. Prompt referral to a cardiac catheterization laboratory with a 24-hour on-call service is therefore necessary.
